An online, open, 42-question survey on Qualtrics with purposive and convenience sampling methods was conducted between May 9 and June 9, 2023 on participants self-identifying as having HFS. Participants were recruited through social media platforms. Only 58.0% of participants reported their HFS symptoms began following a specific incident/injury. Changes in penis shape/size (92.3%) and rigid penis when not erect (90.9%) were the most common complaints. Activities such as laying down and stretching improved symptoms in 73.0% and 44.1% of the participants, respectively, while masturbation and standing worsened symptoms in 75.9%, and 64.5% of the participants, respectively. Pudendal neuralgia (16.9%) was the most prevalent comorbid condition. Of those who participated in therapies, phosphodiesterase-5 (PDE5) inhibitor treatment had the highest patient global impression of change (PGIC) score (2.6 ± 1.1), indicating little to moderate improvement in symptoms. All other therapies scored between 1 and 2, indicating no change to little improvement in symptoms: pelvic floor physical therapy (PFPT) (1.8 ± 0.9), shockwave therapy (1.6 ± 1.1), diet/nutrition changes (1.6 ± 0.8), nerve blocks (1.6 ± 0.8), muscle relaxants (1.5 ± 0.6), anti-inflammatory medications (1.5 ± 0.7), cognitive therapy (1.4 ± 0.7), and nerve pain medications (1.4 ± 0.5). Overall, a direct injury to the penis may not necessarily be the only cause of HFS for some patients, and current therapies generally do not benefit most patients. Still suffering from chronic pain after a total hysterectomy (43 years old) and most visible lesions removed. Hello thanks for your video I just give some information to help people who like me, have persistent / permanent genital arousal. Here are the main causes of PGAD, the solutions and the examinations to be done to obviously cure it! Either the cause of PGAD is psychological: Big emotional shock or trauma following an accident or a physical aggression, rape, recent or even very old touching = during childhood for example! Follow a good therapy => hypnosis or the EMDR technique which is effective! (avoid if possible the antidepressant drug treatments!...) Otherwise often the main causes of PGAD are => A Tarlov cyst or a hernia or a disc protrusion which compresses or irritates certain nerve roots (nerves) coming from the Sacro-lumbar vertebrae of S1, S2, S3 and S4 which go to the genital organs! Or often a cyst that compresses (pinches) a part of the Pudendal nerve which is the great nerve + its 3 branches of the genitals ! It is enough that a root (nerve) is irritated or compressed for that to cause a permanent genital excitation ! Other causes: Sometimes it is due to a neurological disease => Polyneuropathy (lesions) of the pelvic nerves or at the level of the pudendal nerve! (Especially at the level of the dorsal nerve of the clitoris, root coming from the pudendal nerve !) Genital pelvic varicose veins are the most common cause of Pgad, accounting for around 55% of cases, especially in women who have had children, and sometimes also in young, childless women. Another cause is vulvodynia. Or sometimes an inflammation of the ligaments of the large gluteal muscle, the Piriformis ! The ligaments (canal d'alcock) of this large muscle are very close to the pudendal nerve and can touch a part of the pudendal nerve and create a PGAD! Consult a good physiotherapist or osteopath ! Perform a perineal electromyography to detect whether a nerve is affected, compressed by a cyst or protrusion, or an injury causing neuropathy. Examinations depending on the case or symptoms of PGAD: an MRI (magnetic resonance image) of the thoracic spine and a pelvic MRI or pelvic Doppler ultrasound. It is best to consult a good pelvic urologist or a good neurologist. For Doppler ultrasound or pelvic ultrasound see an angiologist or your doctor. Do not be discouraged! There are solutions to cure and stop PGAD! Sincerely 🌷

Thank you for your video . I have suspected pundenal neuralgia after a sacrum epidural nerve block for my back and pin and needles in my saddle regoin. After thr block I was way worse and I was getting sharp nerve pain when I would go to thr toilet, I can now not sit at all, nor lie on my left side or lie flat on my back, I can only lie on my right. Left glute pain is triggered by lying flat on my back or touch . In my saddle regoin I get nerve pain and all that area down below. Does glute pain that's trigger by lying flat or touch or by sitting correspond with pundendal neuralgia?

You are a tremendous tberapist. I had issues in my pelvic area that involvewd my bowels being loose constantly. I went to a pelvic floor specialist and she solved my problem. My primary suggested the pelvic floor speicialist after medication was not helping the problem. I am still doing the kegels to keep my pelvic floor strong, once daily. Is there anyone you can recommend in san diego area who specializes in PN or PNE? I've been suffering for 6 months from a horrible pap smear injury that injured my pudendal nerve. I've seen 2 physical therapists and 4 different doctors and the doctors don't seem to know much about it and they are either saying there are no imaging tests I can get done or they are honest and tell me they don't know what to order. They just keep referring me to different doctors. I'm on several meds for the pain, and it doesn't seem to be helping. I'm suffering a great deal, and any help you can provide would be very much appreciated. Thank you.

I find that drinking hot tea gives some mild relief. I used to take co-codomol but by the time it kicked in the pain was subsiding so it wasn’t instant enough plus I don’t like the groggy feeling it gives me. It’s definitely associated to stress and tiredness.
